You will write a check personally and directly payable to the NannyNurse ™ based on the agreed time listed on the Wellness log. The NannyNurse ™ will calculate the NannyNurse™ fee according to the effective Child Care Fee Schedule, rounded to the next quarter hour. Your check number and amount will also be recorded on the Wellness Log. This is your receipt. |

When you arrive home, the NannyNurse ™ will record the time, initial and ask you to initial. They will review the Wellness Log and answer any questions about the child’s day. Remember, NannyNurses™ do not give medical advice. The top copy of the Wellness Log is for your records. |

If any symptoms cause concern, the NannyNurse™ will immediately call you so you can call the doctor. In case of an emergency, Member agrees to come home or go to the hospital designated promptly upon a call from NannyNurse™ who will call 911 first upon a condition deemed urgent. Children will not be transported by a NannyNurse™ to a medical facility. |

The NannyNurse™ will monitor your child’s symptoms and well being throughout the day and record observations in the Wellness Log hourly. They will follow you and your doctor’s directions that you put in writing on the Wellness Log. They will prepare and serve meals and snacks that you provide and provide companionship for your child. Your NannyNurse™ will call you to check in during the scheduled times listed on the Wellness Log. |

The NannyNurse™ will arrive at the agreed time. (If they are more than 15 minutes late, there is no charge for the first hour. The NannyNurse™ will note the arrival time on the Wellness Log, initial, and ask you to initial so there is agreement.)
The NannyNurse™ will thoroughly complete and review the top half of the Wellness Log with you, including the times to call you with updates (every 3 hours for example). The NannyNurse™ will also review your child’s current symptoms, medication instructions, allergies, special needs, mealtime/snack time instructions and suggested comfort measures. All available NannyNurses™ will be contacted upon your request. After the NannyNurse™ accepts the placement; you will receive a phone call confirming the details (date, time, duration, location, needs of child). |

When your child is sick and you need to work, log on to www.NannyNurseNetwork.com. Enter your username and password to access your account. Select “Request a NannyNurse™” and enter in all of the important information that the NannyNurse™ will need to best care for your child. |

Log on to www.NannyNurseNetwork.com and select “Join Now!” You will enter important information about your family so we can provide the best possible care for your child’s needs. When you pay for your Enrollment Fee, enter a promotion code, if applicable. You now have access to NannyNurse™ sick childcare services for your family. |

Parents know when their child is too sick to perform well at school, is contagious or needs extra rest to heal. Also, children cannot attend daycare or school unless they have been fever free (under 100 degrees) without medication for 24 hours. |

No. NannyNurse™ Network LLC does not provide health care, nursing services or other medical care. |

All Network fees are subject to change periodically by the Company. Such changes will be published on the website. No increases will be applicable to any Member during their enrollment year. |

Unless a minimum of 8-hours notice of cancellation is provided by Member, the Member’s credit card on file will be charged for the Placement Fee ($15) which reimburses the Company for administration of the placement and a 4-hour minimum NannyNurse™ hourly fee ($56.00). This hourly fee reimburses the NannyNurse for accepting the assignment, which makes the NannyNurse unavailable for alternative assignments. |

We currently accept MasterCard and Visa for the Enrollment Fee and Placement Fee. Payments to NannyNurses for services rendered should be made directly by check or cash. While participation in the Network anticipates no issues with payments to the Network or the NannyNurse serving you, you authorize the Network to charge your credit card the Childcare Hourly Fees together with a $50.00 administrative charge for any check to a NannyNurse™ returned for insufficient funds. |

There are three kinds of fees in the Network. First, there is the nonrefundable Annual Enrollment Fee in the amount of $75.00, which is paid online by credit card upon joining the Network. Second, each time you request a NannyNurse™ for a specific assignment there is a $15.00 Placement Fee which will be paid online by credit card. At the end of a daily assignment (maximum 12 consecutive hours) NannyNurses™ are paid directly by the Members the Childcare Hourly Fee of $14.00 per hour for Tier One, rounded to the next quarter hour. There is a minimum of 4-hours for each placement. (Each additional child is $2 per hour.) |
