What you need to submit prior to Boot Camp:

• Completed Application (attachment at bottom)
• Current CPR certification
• Copy of your current certifications or licenses:
  • CNA
  • Medical Assistant
  • EMT
  • LPN
  • RN
• Copy of your current driver’s license
• 3 valid professional references (Please provide current contact information)
• Background Check Release Form (attachment at bottom)

 What you need to bring with you to Boot Camp:

• Copy of Liability Insurance Policy for your own protection. If you currently do not have this, we will have information available for you.
• Please wear a pink scrub top for your photo for our web site.
• Signed NannyNurse™ Network Enrollment Agreement (provided at Boot Camp)
